We do not accept insurance and are considered an "out of network" provider. Payment is expected at the time of your visit. You will receive a very detailed superbill encounter form to submit to your insurance carrier for reimbursement under their "out of network provider" benefits. This will contain all the information necessary to process your claim. We do not submit to your insurance company on your behalf.
Because we are a low volume, highly patient attentive/ “high touch and personalized” practice and see a limited number of patients, and our appointments are at a premium, we require your credit card information up front to make an appointment. The card will not be run until you complete your visit or no show or cancel with less than 24 hours notice.
The educational and other activities at Women Wise are offered through another company, Enlightened Women Rising, and are part of the whole package concept of the Holistic Women’s Health and Wellness Practice and pair with one another and are not separable. In other words, to be a patient at Women Wise, involves a separate reasonable yearly association/ concierge fee with Enlightened Women Rising. These fees cover educational seminars, speakers, activities and refreshments whether you utilize them or not. The weekly activities/lectures/classes are part of being a primary care patient of Women Wise. Non patients may attend an activity or lecture if they are the guest of an existing patient and a small nominal fee is required. The activities are not open to the general public although on occasion, there may be a specifically scheduled activity for marketing, recruitment and promotional purposes. Clients of healing arts providers or other associated clinicians are not eligible to attend unless they become a primary care patient at Women Wise, allowing them full access to all activities.
Because we are so small, low volume and spend extended time during our visits, we require 24 hours notice for cancellation so we can offer the appointment to others that need one. If cancellation occurs with less than 24 hours notice, the credit card you have uploaded will be charged for the entire visit. We are reasonable and exceptions may be made for family emergencies. We accept personal checks and credit card payments for office visits. We do not keep credit card information on file and you will be required to upload your credit card information, each time you make an appointment. We do accept and bill insurance for lab work and other diagnostic tests such as cultures and pap smears.
Because we wish to offer an exceptional and personal health experience beginning from the minute you walk through our doors, we require that your medical history and forms be filled out no later than 48 hours before your appointment otherwise your appointment will be cancelled. This allows Eve to review your history well in advance, plan accordingly and be able to focus on you immediately and allow you and she to make the most of your time here at Women Wise. Additionally, ONE HOUR before your visit, you will receive a review of systems history form from our electronic medical record company that needs to be filled out on your phone or device, prior to your visit. This unfortunately cannot be sent any sooner with the other medical records required to be completed 48 hours before your visit. Please plan accordingly and arrive 15 minutes early to fill this form out in your car or the waiting room on your cell phone or device. Our desire is to serve motivated women wanting a comprehensive, attentive, different, intimate, and superb health care visit.
